Getting a new hospital ID card for new patients
• Contact room #100 on the 1st floor to get a new hospital ID card.
• Fill in the application form and submit it with your identification card.
• Contact a nurse to screen your symptoms.
• Contact a medical record staff to check the correctness of your details.
• After completion, you will be advised to the examination unit according to your symptoms.
Searching for your old medical records
• Hand in your hospital ID card and inform our staff of your symptoms.
• Get informed about the examination room from the medical record officer.
Service time
• Monday - Friday 07:00 - 11:00 except public holidays.
• After 12:00 noon this service is offered to emergency patients only. Please contact the nurse in front of the emergency room.
Remark: - Please show your personal ID every time you contact the hospital.
